Note: Vault prepopulates fields on the Disclosure Form when a user or Vault creates the Disclosure record. The fields remain unchanged unless manually updated, even if the original data changes.
The table below lists the prepopulated _Disclosure Form_ fields and the field the data populates from. For example, Vault will populate the _Unique Protocol Identification Number_ field using the value from the _Study Number_ field on the selected _Study_ record.

### Site List
Vault populates the _Site List_ section with _Study Site_ data from the selected _Study_. The _Study Site_ data is read-only in the _Site List_ section. You can locate the *Site List* in the _More Info_ section of the _Disclosure Form_.
_Study Sites_ populate into the _Site List_ based on the sites marked as _Include for Disclosure_. Vault automatically selects the _Include in Disclosures_ checkbox on all new _Study Site_ objects. To have a _Study Site_ populate into the _Disclosure_ record, ensure the _Include in Disclosures_ checkbox is selected while creating or editing a _Study Site_.
_Principal Investigator_ data for disclosed _Study Sites_ also populates into the _Site List_. Unlike _Study Sites_, Vault does not automatically select the _Include in Disclosure_ checkbox on the _Study Person_ record for the selected _Principal Investigator_. Select the **Include in Disclosures** checkbox when editing the _Study Person_ record for the _Principal Investigator_ to ensure Vault populates the data in the _Disclosure_ record.
Vault populates the _Site List_ section from Clinical Operations records. The following table lists the populated *Disclosure Form* record fields and the Clinical Operations object and fields from which the data populates:
